If you have the original certificate you must return it to us in the post.
If you need a replacement certificate because of a change of name, you will also need to upload a certified copy of your change of name document e.g. marriage certificate/change of name deed.
All documents must be certified as a true copy of the original by either your employer or by another professional such as a Doctor, Teacher, Solicitor, Lawyer, Banker or Police Officer.
You will need to take a copy of the original document and the 'certifier' should:
- State the following on the copy of the document, ‘I have seen the original document and I certify that this is a complete and accurate copy of the original’
- Write their name, position and/or capacity (eg lawyer/employer), contact address and telephone number
- Add their signature and date
